How It Works
If you’re enrolled in a UMR or Meritain Health medical plan, RxCrossroads by McKesson will cover the following prescription drugs at no cost to you:
- Afstyla®
- Hizentra®
- Idelvion®
- Zemaira®
To fill a prescription for one of these CSL products, ask your doctor to complete the applicable Employee RxCrossroads Drug Form (located on Benefits In-Site) and fax it, along with copies of your prescription and insurance ID card, to RxCrossroads by McKesson at 833-541-0173. If you need assistance, you can speak with a pharmacist at RxCrossroads by calling 855-393-3617, Monday–Friday, 9 a.m.–6 p.m. ET.
If you’re enrolled in a Kaiser plan, all CSL products, including the above four products, are covered according to rules established by Kaiser. For additional information, contact Kaiser directly.
